Zocor (Simvastatin) is a cholesterol-lowering drug. Your doctor may prescribe Simvastatin in addition to a cholesterol-lowering diet if your blood cholesterol level is too high, and if you have been unable to lower it by diet alone. Zocor description

Why is this drug prescribed?
Zocor is a cholesterol-lowering drug. Your doctor may prescribe Zocor in addition to a cholesterol-lowering diet if your blood cholesterol level is too high, and if you have been unable to lower it by diet alone. For people at high risk of heart disease, current guidelines call for considering drug therapy when LDL levels reach 130. For people at lower risk, the cut-off is 160. For those at little or no risk, it's 190.
In people with high cholesterol and heart disease, Zocor reduces the risk of heart attack, stroke and "mini-stroke" (transient ischemic attack) and can stave off the need for bypass surgery or angioplasty to clear clogged arteries.

Most important fact about this drug:
Zocor is usually prescribed only if diet, exercise, and weight-loss fail to bring your cholesterol level under control. It's important to remember that Zocor is a supplement to—not a substitute for—those other measures. To get the full benefit of the medication, you need to stick to the diet and exercise program prescribed by your doctor. All these efforts to keep your cholesterol levels normal are important because together they may lower your risk of heart disease.

How should you take this medication?
Take Zocor exactly as prescribed.

- If you miss a dose...
Take it as soon as you remember. If it is almost time for your next dose, skip the one you missed and go back to your regular schedule. Do not take 2 doses at once.

Store at room temperature.

What side effects may occur?
Side effects cannot be anticipated. If any develop or change in intensity, inform your doctor as soon as possible. Only your doctor can determine whether it is safe for you to continue taking Zocor.

Why should this drug not be prescribed?
Do not take Zocor if you have ever had an allergic reaction to it or are sensitive to it.
Do not take Zocor if you have active liver disease.
Do not take Zocor if you are pregnant or plan to become pregnant.

Special warnings about this medication:
Because Zocor may damage the liver, your doctor may order a blood test to check your liver enzyme levels before you start taking the drug. Blood tests will probably be done before your treatment is started and at periodic intervals for a year after your final dosage increase. If your liver enzyme levels rise too high, your doctor may tell you to stop taking Zocor.

Since Zocor may cause damage to muscle tissue, be sure to tell your doctor of any unexplained muscle tenderness, weakness, or pain right away, especially if you also have a fever or feel sick. Your doctor may want to do a blood test to check for signs of muscle damage.
If you are scheduled for major surgery, your doctor will have you stop taking Zocor a few days before the operation.

Possible food and drug interactions when taking this medication:
Zocor tends to enhance the effects of the blood-thinning drug Coumadin and the heart medication Lanoxin. Combining it with the following drugs increases the chance of muscle damage:
Amiodarone (Cordarone)
Clarithromycin (Biaxin)
Clofibrate (Atromid-S)
Cyclosporine (Sandimmune, Neoral)
Erythromycin (PCE and others)
Fenofibrate (Tricor)
Gemfibrozil (Lopid)
Itraconazole (Sporanox)
Ketoconazole (Nizoral)
Nefazodone (Serzone)
Nicotinic acid or niacin (Niaspan)
Protease inhibitors (used in the treatment of HIV), including Agenerase, Crixivan, Fortovase, Invirase, Norvir, and Viracept
Verapamil (Calan)
If you are taking Zocor with any of these drugs (or with large quantities of grapefruit juice) alert your doctor immediately at the first sign of muscle pain or weakness. If you need to take erythromycin, Biaxin, Nizoral, or Sporanox, the doctor may temporarily take you off Zocor.

Special information if you are pregnant or breastfeeding:
You must not become pregnant while taking Zocor. This drug lowers cholesterol, and cholesterol is needed for a baby to develop properly. If you do become pregnant while taking Zocor, notify your doctor right away. Based on studies of other cholesterol-lowering drugs, it is assumed that Zocor could appear in breast milk and could cause severe adverse effects in a nursing baby. Do not take Zocor while breastfeeding your baby.

Recommended dosage:
You will have to follow a standard cholesterol-lowering diet before starting treatment with Zocor and continue this diet while using Zocor.

All doses should be adjusted to your individual needs.

ADULTS:
The usual starting dose is 20 milligrams once a day in the evening. If your cholesterol is especially high, the doctor may start with a dose of 40 milligrams. The dosage can be adjusted every 4 weeks. Some people with severe, hereditary high cholesterol may be prescribed as much as 80 milligrams a day, taken in doses of 20, 20, and 40 milligrams, along with other treatments.
Those who have severe kidney disease should use Zocor with caution. The recommended starting dose is 5 milligrams per day.
When combined with cyclosporine, niacin, Atromid-S, Lopid, or Tricor, the dosage of Zocor should not exceed 10 milligrams a day. When combined with Calan or Cordarone, the dosage of Zocor should not exceed 20 milligrams a day.

Overdosage:
Although no specific information about Zocor overdose is available, any medication taken in excess can have serious consequences. If you suspect an overdose of Zocor, seek medical attention immediately.

Zocor notes:

Zocor generic equivalent simvastatin is a cholesterol-lowering medication that blocks the production of cholesterol in the body. Simvastatin reduces cholesterol and total cholesterol in the blood. It is used in adults and children (10 years of age and older).

Zocor side effects include abdominal pain, headache, muscle tenderness, weakness, pain, constipation, fever or flu symptoms. Before starting medication, it is necessary to consult doctor if patient is allergic to any drugs or have diabetes, underactive thyroid, kidney disease or muscle disorder, also Zocor may damage the liver so it is recommended to have blood test to check liver enzyme levels. Zocor with grapefruit and grapefruit juice can lead to potentially dangerous effects.

Avoid drinking alcohol while taking Zocor. Alcohol can raise triglyceride levels, and may also damage liver. Avoid eating foods that are high in fat or cholesterol. Zocor effect will not be effective in lowering cholesterol if a cholesterol-lowering diet plan is not followed.

If drugs like Erythromycin, Cyclosporine or Fenofibrate taken with Zocor can cause muscle pain. Consult doctor if pain gets worsened.

The recommended Zocor dose is 20 to 40 mg or as prescribed by doctor. Do not take the medication in larger amounts, or take it for longer than recommended by doctor. Take Zocor with a full glass of water, usually it is taken at bedtime or with an evening meal. Patients with kidney disease should use Zocor with caution. The recommended starting dose is 5 mg per day. Girls must have been menstruating for at least 1 year before starting therapy with Zocor.

You must not become pregnant while taking Zocor. This drug lowers cholesterol, and cholesterol is needed for a baby to develop properly. If you do become pregnant while taking Zocor, notify the doctor right away. Based on studies of other cholesterol-lowering drugs, it is assumed that Zocor could appear in breast milk and could cause severe adverse effects in a nursing baby. If an overdose of Zocor is suspected, seek medical attention immediately. An overdose of Zocor is not expected to produce life-threatening symptoms. If a dose is missed, it is advised to consume it as soon as it is remembered. If it is near the time of the next dose, the missed one should be skip and the usual dosing schedule should be resume.

Store Zocor at room temperature (between 41 and 86 degrees F (5 and 30 degrees C), protected from moisture, heat, and light.

Zocor news:

Recently a study was conducted to systematically review randomized trials comparing statins to placebo for heart failure(HF) and compare the impact of different statins. Although statins have been shown to improve outcomes in retrospective analyses of patients with heart failure, recent randomized placebo-controlled trials have shown mixed results. Different case study materials were collected from reliable directories like CENTRAL, mRCT and PubMed. Grouping of resources was performed with random effect methods along with summary effect estimates (95% confidence intervals).

Ten studies (10,192 patients) with follow-up from 3 to 47 months were included. The patients were distributed as in group of rosuvastatin, 1 to zocor(simvastatin) and 6 to atorvastatin. It was revealed that statins did not affect all-causes of heart failure or cardiovascular mortality but did significantly decrease hospitalization for worsening HF during follow-up (odds ratio 0.67).

Patients distributed to statins had a significant 4.2% increase in left ventricular ejection fraction(LVEF) at follow-up (95% confidence interval 1.3 to 7.1). Heterogenity was seen among different statins in the post analysis. It was demontrated from the study that distribution to atorvastatin group significantly decreases all-cause mortality and also reduces the rate of hospitalization for worsened HF. Atorvastatin and zocor(simvastatin) distribution led to a significant improvement in LVEF, whereas these benefits were not observed in patients randomized to rosuvastatin. Thus it was concluded that that statins are safe and they help in improvement of LVEF there by decreasing the hospitalization rate for heart failure conditions.


Zocor story:

Zocor s a lipid-lowering agent that is derived synthetically from a fermentation product of aspergillus terreus. After oral ingestion, simvastatin which is an inactive lactone is hydrolyzed to the corresponding â-hydroxyacid form. It is used to control hypercholesterolemia (elevated cholesterol levels) and to prevent cardiovascular disease. In patients with coronary heart disease (CHD) or at high risk of CHD this medication can be started simultaneously with diet.

In the pre-marketing controlled clinical studies and their open extensions 1.4% of patients were discontinued due to adverse reactions. The most common adverse reactions that led to treatment discontinuation were gastrointestinal disorders (0.5%), myalgia (0.1%) and arthralgia (0.1%). The most commonly reported adverse reactions (incidence 5%) in zocor controlled clinical trials were upper respiratory infections (9.0%), headache (7.4%), abdominal pain (7.3%), constipation (6.6%), and nausea (5.4%).

Zocor is a lactone that is readily hydrolyzed in vivo to the corresponding â-hydroxyacid, a potent inhibitor of HMG-CoA reductase. Inhibition of HMG-CoA reductase is the basis for an assay in pharmacokinetic studies of the â-hydroxyacid metabolites and following base hydrolysis, active and latent inhibitors in plasma following administration of zocor. Epidemiological studies have demonstrated that elevated levels of total-C, LDL-C as well as decreased levels of HDL-C are associated with the development of atherosclerosis and increased cardiovascular risk. Lowering LDL-C decreases this risk. However, the independent effect of raising HDL-C or lowering TG on the risk of coronary and cardiovascular morbidity and mortality has not been determined. In a study including 16 elderly patients between 70 and 78 years of age who received zocor 40 mg/day, the mean plasma level of HMG-CoA reductase inhibitory activity was increased approximately 45% compared with 18 patients between 18-30 years of age. Clinical study experience in the elderly suggests that there were no overall differences in safety between elderly and younger patients.

In the Multicenter Anti-Atheroma Study, the effect of zocor on atherosclerosis was assessed by quantitative coronary angiography in hypercholesterolemic patients with CHD. In this randomized, double-blind and controlled study, patients were treated with zocor 20 mg/day or placebo. Angiograms were evaluated at baseline of two and four years. The study endpoints were mean change per-patient in minimum and mean lumen diameters, indicating focal and diffuse disease, respectively. Zocor significantly slowed the progression of lesions as measured in the year 4 angiogram by both parameters, as well as by change in percent diameter stenosis. In addition, zocor significantly decreased the proportion of patients with new lesions and with new total occlusions.

Zocor has been known to be effective in reducing total-C and LDL-C in heterozygous familial and non-familial forms of hyperlipidemia and in mixed hyperlipidemia. Maximum response is generally achieved within 4-6 weeks and maintained during chronic therapy. This medication consistently and significantly decreased total-C, LDL-C, total-C/HDL-C ratio and LDL-C/HDL-C ratio. It also decreased TG and increases HDL-C.

An experimental study was carried out on 160 rats to observe the improvement in vestibulomotor function and in reduction of cognitive dysfunction. The effects of zocor on vasospasm and regional cerebral blood flow (rCBF) were explored. Effects of 10 mg/kg/d,1.5 mg/kg/d, morris water maze, neuronal survival, cerebral arterial diameter and rCBF were determined. A dose dependent response to zocor was observed with more rapid improvement in vestibulomotor function, less basilar arterial vasospasm and improved cortical neuronal survival. However, rotarod performance in the group of 10 mg/lg/d were deteriorated after 1 week, which correlated with the increased plasma creatine kinase levels. Furthermore, when zocor was discontinued after 2 weeks, the usual treatment duration in SAH clinical trials, rotarod performance deteriorated acutely, rCBF returned to control values and no long-term benefit was observed in terms of visual spatial memory. Continuing zocor 1.5 mg/kg/d for 5 weeks resulted in sustained improvement in rotarod performance, reduced escape latency, swimming distance and swimming speed versus vehicle. The results indicated that long-term cognitive dysfunction after experimental SAH in the rat can be reduced by the use of zocor.
