Tadalafil is a PDE5 inhibitor, currently marketed in pill form for treating erectile dysfunction (ED) under the name Cialis; it has recently been approved for the treatment of pulmonary arterial hypertension, and has been used for other conditions. Cialis tablets, in 5 mg, 10 mg, and 20 mg doses, are yellow, film-coated, and almond-shaped. The approved dose for pulmonary arterial hypertension is 40 mg, and will be marketed under the brand name Adcirca. In December 2003, the Food and Drug Administration approved tadalafil (as Cialis) for sale in the United States as the third ED prescription drug pill (after sildenafil citrate (Viagra) and vardenafil (Levitra)). Cialis's 36-hour effectiveness earned it the nickname, "The Weekend Pill"; like sildenafil and vardenafil, tadalafil is recommended as an 'as needed' medication. Cialis is the only one of the three that is also offered as a once-daily medication.

Drug Mechanism: During sexual stimulation, Cialis enables a man's penis to fill with enough blood to cause an erection. After sex is over, the erection goes away. You will not get an erection by just taking Cialis. Sexual stimulation is required for Cialis to be effective. Your doctor or health care professional is always your best source of information when evaluating treatments for erection problems.
Dosage Form: Cialis is commonly sold in 10 mg and 20 mg strengths. For most patients, 10 mg is sufficient to achieve effective results. Doctor's usually start their patients on 10 mg and increase the dose if required. If 10 mg is sufficient for you, it may be more economical to purchase the 20 mg strength and split the pill the pill into 2 pieces of equal size with a pill cutter (subject to the approval f your doctor). Taking more Cialis than is prescribed is unlikely to provide additional benefits and may in fact be harmful.
Drug Interactions: Cialis can cause your blood pressure to drop suddenly to an unsafe level if it is taken with certain other medicines such as nitrates and alpha-blockers, and recreational drugs that contain nitrates called "poppers". A sudden drop in your blood pressure could cause you to become dizzy, faint, or have a heart attack or stroke.
Pregnancy: Cialis is only for men with ED. Cialis is not for women or children. Cialis has not been studied in pregnant women. Cialis has not shown to cause birth defects or other problems in animals.
Nursing Mothers: Cialis is only for men with ED. Cialis is not for women or children. It is not known whether Cialis passes into human breast milk.
Alcohol Considerations: Cialis works just as well with or without a meal and is effective even with moderate alcohol intake. Although, alcohol can make the side effects from tadalafil worse. When taken in excess, alcohol can increase your chances of getting a headache or getting dizzy, increasing your heart rate, or lowering your blood pressure. Your doctor will tell you how much alcohol you may drink while you are taking this medication.
Possible Side Effects: Like all prescription medications, Cialis may produce some side effects. These side effects are generally mild to moderate in nature, and usually go away within a few hours of use. They include: headache, indigestion, back pain, muscle aches, flushing, stuffy or runny nose.
These side effects usually go away after a few hours. Patients who get back pain and muscle aches usually get it 12 to 24 hours after taking Cialis. Back pain and muscle aches usually go away by themselves within 48 hours.
Storage: Keep this medication in the container it came in, tightly closed, and out of reach of children. Store it at room temperature 59-86°F (15-30°C) and away from excess heat and moisture (not in the bathroom). Throw away any medication that is outdated or no longer needed. Talk to your pharmacist about the proper disposal of your medication.
