Nifedipine (brand name Adalat, Nifedical, and Procardia) is a dihydropyridine calcium channel blocker. Its main uses are as an antianginal (especially in Prinzmetal's angina) and antihypertensive, although a large number of other uses have recently been found for this agent, such as Raynaud's phenomenon, premature labor, and painful spasms of the esophagus in cancer and tetanus patients. It is also commonly used for the small subset of pulmonary hypertension patients whose symptoms respond to calcium channel blockers.

PRESCRIBED FOR:
Chest pain (angina) occurs because of insufficient oxygen delivered to the heart muscles. Insufficient oxygen may be a result of coronary artery blockage or spasm, or because of physical exertion which increases heart oxygen demand in a patient with coronary artery narrowing. Nifedipine is used for the treatment and prevention of angina resulting from coronary artery spasm as well as from exertion. Nifedipine is also used in the treatment of high blood pressure. It is also used to open the blood vessels which spasm, causing Raynaud's phenomenon.
DOSING:
Nifedipine can be taken with or without food. Nifedipine is metabolized mainly by the liver and dosages may need to be lowered in patients with liver dysfunction.
DRUG INTERACTIONS:
In patients with severe obstructive coronary artery disease, nifedipine can increase the frequency and severity of angina or actually cause a heart attack on rare occasions. This phenomenon usually occurs when first starting nifedipine, or at the time of dosage increase. Excessive lowering of blood pressure during initiation of nifedipine treatment can occur, especially in patients already taking another blood pressure lowering medication. In rare instances, congestive heart failure has been associated with nifedipine, usually in patients already on a beta blocker.
Administration of nifedipine with digoxin can increase digoxin blood levels. Therefore, blood levels of digoxin are usually monitored to avoid toxicity. Concurrent administration of cimetidine (Tagamet) interferes with the liver breakdown of nifedipine, and significantly increases nifedipine blood levels. Therefore, cautious dosing is necessary when both medications are administered. Generally, nifedipine is avoided in children.
NURSING MOTHERS:
Generally, nifedipine is avoided in nursing mothers.
SIDE EFFECTS:
Side effects of nifedipine are generally mild, and reversible. Most side effects are expected consequences of the dilation of the arteries. The most common side effects of nifedipine include headache, dizziness, flushing, and edema (swelling) of the lower extremities. Less common side effects include dizziness, nausea and constipation.

Available with the product brand name: Procardia, is a highly effective drug prescribed for treating the condition of severe chest pain known as angina pectories. Adalat works by relaxing the coronary arteries, thereby helping to prevent chest pains that are caused as a result of coronary artery spasm. Since coronary artery blockage or spasm is caused as a result of insufficient oxygen supply to the heart, adalat (Procardia) prevents physical exertion and also controls high blood pressure. It also helps in opening the blood vessels that are affected by the spasm caused by Raynaud's phenomenon.
Since adalat is metabolized primarily by the liver, patients affected by liver dysfunction should avoid this medicine. Note that consuming grapefruit along with this drug should also be avoided to prevent adalat grapefruit interaction that can increase the effect of the medication. Taking adalat during pregnancy is also prohibited. Most importantly, it is prohibited to consume alcohol while taking this drug because of the side effects related to adalat and alcohol. It may increase drowsiness and dizziness along with the decrease of blood pressure.
You should also be aware of various adalat side effects that can occur such as dizziness, headache, flushing, edema (swelling) of the lower extremities, dilation of the arteries etc. Some rare side effects of adalat include nausea, dizziness, and constipation. People suffering from obstructive coronary artery disorder should also avoid taking this drug as in rare cases it is found that it can increase the severity and frequency of angina. However, in certain people reduction of blood pressure may occur in the initial stages. Therefore, it is recommended to use the drug only after proper medical consultation.
